Problems with TOCs if sections have titles that are the same
We are finding that repeated titles in a doc messes up the TOC. See attached doc.

Hi Brad,
just a short update. Today I fixed this issue as much as it is possible.
More specifically:
- Duplicate headings will correctly appear in the TOC and PDF bookmarks
- Links from the TOC and PDF bookmarks to the sections will work correctly to each duplicated section
- Links from the content to a duplicated section will always link to the first occurrence
Unfortunately I can't fix that last issue, as this is the same behavior as in Confluence. The problem is that Confluence does not create distinct IDs (page anchors) for repeating headings on the same page. Therefore when clicking a link the Web Browser (and also the PDF viewer) picks the first one it can find
